Support at Home

Moyne Shire Council in a government-approved provider for aged care services under the Support at Home program.

Our dedicated team of care partners are based in our Port Fairy and Mortlake offices are ready to help you develop your care plans tailored to your needs and goals to help you remain living at home and in the community safely and independently.

We have a team of care workers based right across the shire ready to deliver services in your home or help you remain active in the community.

What is Support at Home

Support at Home was introduced on 1 November 2025 and replaces the Home Care Packages Program. It is a program to help older people remain at home as they age by delivering coordinated care and services to meet their assessed ageing related care needs.

The service is funded by the Australian government and clients may have to make a contribution based on a means test.

Support at Home is provided at eight classifications with an assessment will determine the classification you receive and the amount of care and services that can be provided

Eligibility and access

Step 1

  • Register with My Aged Care Visit the My Aged Care website or call 1800 200 422 to register your need for support.
  • You can also have a family member or carer register on your behalf. 

Step 2

  • Get an assessment after registering, you will be referred for an aged care assessment through the single assessment system. 
  • An assessor will visit you at home (or conduct an online/phone assessment) to understand your care needs and determine your eligibility for government funding. 

Step 3

  • Receive your funding classification and plan based on the assessment, you will be assigned a classification level, which determines the amount of funding available. 

Step 4

  • Choose a provider and start services once your classification is confirmed, you can choose a government-approved provider to deliver your in-home services.

Support at Home cost and fees

You will need to contribute towards the cost of your services, based on the service being delivered and what you can afford. You may also have to provide information about your income and assets to Services Australia to work out exactly how much you will be asked to contribute.

Under Support at Home, you only pay contributions on the services you have received. You won’t contribute to any clinical support costs. More information about fees and costs can be found here.

A 10 percent care management fee will be deducted from your quarterly budget.

Through your Support at Home funding, you can access a range of services which are tailored and coordinated specifically to meet your individual care needs with the support from your care partner through your.

Council's dedicated team of locally based care workers and our trusted associate providers can deliver the following services:

  • Respite care and companionship
  • Emergency in-home respite
  • Home help, including meal preparation

  • Transport to appointments including taxi cards

  • Home and garden maintenance
  • Communication aids assistance
  • Mobility equipment purchases and leasing
  • District nursing
  • Meals delivery
  • Home modification 

Your rights

The rights and responsibilities of recipients of home care packages and their providers are determined by the Australian government. Your home care package client handbook and home care agreement contains written information on advocacy services and complaint mechanisms.

You can also access the  Statement of Rights which describes your legislated rights you have while accessing aged care services.
