Home Care Packages (HCP) Program

The HCP Program supports senior Australians with complex care needs to live independently in their own homes, using a consumer-directed care approach to ensure the support suits a person’s needs and goals. Home care packages (‘packages’) deliver co-ordinated packages of care and services to meet people’s assessed care needs within the limits of their individual home care budget and the scope of the Program.

The service is funded by the Commonwealth Government and Home Care Packages are provided at four level which include:

  • Level 1: Basic level of care needs - $9,179.75 a year
  • Level 2: Low level of  care need -   $16,147.60 a year
  • Level 3: Intermediate level of care need -  $35,138.55 a year
  • Level 4: High level of care needs - $53,268.10 a year

The main difference between these levels is the amount of care and services that can be provided, rather than the type of care.  The care and services provided ranges from basic care and support needs through to high level care and support.

Eligibility and Access

To receive a Home Care Package you must be referred to the Aged Care Assessment Team (ACAT) by your GP, district nurse, hospital, health professional, or you can refer yourself through My Aged Care , or call  1800 200 422. Assessments are at no cost.

When assessed, there can be a wait time until a Home Care Package becomes available. Client will received a letter from My Aged Care when a package is available to let you know that you have been assigned a package.  The letter will set out the Home Care Package level and include a unique referral code. You have just 56days to start using your package or you may lose your funding.

HCP Cost and Fees

If you are eligible for a Home Care Package, it’s important to understand what fees you may have to pay, how much the government will contribute, and how your budget work.  Our Home Care Package team offers a very low Management fee of 25% combined, which are deducted from your daily Home Care Package subsidy.

  • 15% Care management fee.
  • 10% Package management fee

Eligible clients who are not full pensioners or self-funded retirees made be required to contribute an Income Tested Care Fee.  This fee is different for everyone and it based on your individual income, including your pension. The Income Assessment is completed through Service Australia.

Our Home Care Packages team do not ask client to contribute Basic Daily Fee

HCP Services

Through your Home Care Package, you can access a range of services which are tailored and coordinated specifically to meet your individual care needs with the support from our Home Care Packages Care Advisors through your individualised Care Plan.

  • Meals delivery
  • Home modification

  • High-level personal care
  • Medication management
  • Wound management
  • Continence management
  • Respite care and companionship
  • Emergency in-home respite
  • Home help, including meal preparation
  • Social activities
  • Transport to appointments including taxi cards
  • Shopping
  • Case management
  • Home and Garden maintenance
  • Communication Aids Assistance
  • Mobility Equipment Purchases and Leasing
  • District nursing
  • Meals delivery
  • Home modification

We also provide an Emergency 24 hour on-call assistance for all our HCP clients.

Your rights

The rights and responsibilities of recipients of Home Care Packages and their providers are determined by the Federal Government. Your Home Care Package Client Handbook and Home Care Agreement contains written information on advocacy services and complaint mechanisms.

You can also access the Charter of Aged Care Rights which describes your legislated rights as a consumer of a Government-subsidised aged care service. These rights apply to all consumers, regardless of the type of care and services they receive.
